I guess the answer is from waking up to going to bed ? but sometimes I work 9-5 then go on call from 21.00 until 09.00 am when I am then back at work again , I always want to eat when I am up all night. Think the answer is not to fast when I am on call

I would not fast on those days.
You are working/on call for 28 hrs, with a 4 hr break !!!!!!!
I don't think its legal in the UK to work for one employer for that length of time.
I seem to remember years ago the hospitals had to stop doing it due EU legistration.
Anyway you must have lots of energy; much more than me

Yes European time directive is a nonsense my longest shift was 37 hours which is a weeks work, I answered my own question really but what I eat etc when , I do these shifts is ridiculous maybe I should aim to be more organised
